Abstract

Prevalensi kejadian penyakit Gagal Ginjal Kronik (GGK) di Indonesia menurut Perhimpunan Nefrologi Indonesia sebesar 25 juta dimana diabetes melitus merupakan penyakit kedua tersering dapat meningkatkan risiko kejadian GGK sebanyak 23% kasus. Penderita GGK membutuhkan lebih banyak pengobatan sehingga hal ini dapat meningkatkan risiko Drug Related Problems (DRPs). Penelitian ini bertujuan untuk mengetahui tingkat kasus DRPs dan tingkat keberhasilan pengobatan pada pasien diabetes melitus dengan GGK. Pengumpulan data dilakukan secara retrospektif berdasarkan catatan rekam medis di RSUD AlIhsan Bandung secara total sampling pada periode Juli-Desember 2023. Berdasarkan hasil penelitian ini terdapat 6 kasus (9,10%) indikasi tanpa terapi, 60 potensi kasus (90,90%) reaksi obat merugikan, dan tidak terjadi kasus pada kategori terapi tanpa indikasi, obat tidak tepat, dosis obat terlalu rendah atau tinggi. Hasil pemeriksaan laboratorium berupa glukosa darah sewaktu, sebanyak 77,78% pasien mencapai target pengobatan dan 22,22% tidak tercapai sedangkan pada kadar ureum dan kreatinin memiliki efektivitas sebesar 55,55 dan sebesar 44,45% tidak menunjukkan efektivitas pengobatan. Kesimpulan dari penelitian ini adalah bahwa terjadi potensi kejadian DRPs pada kategori indikasi tanpa terapi terjadi sebanyak 6 kasus (9,10%) dan interaksi obat terjadi sebanyak 60 potensi kasus (90,90%). Tingkat efektivitas terapi pengobatan berdasarkan parameter kadar glukosa darah menunjukkan 77,78% sedangkan kadar ureum dan kreatinin sebesar 55,55%. ----- The prevalence of Chronic Kidney Disease (CKD) in Indonesia, according to the Indonesian Nephrology Association, is 25 million cases, with diabetes mellitus being the second most common disease, increasing the risk of CKD by 23% of cases. CKD patients require more medications, thereby increasing the risk of DrugRelated Problems (DRPs). This study aims to determine the incidence of DRPs and the success rate of treatment in CKD patients with diabetes mellitus. Data collection was conducted retrospectively based on medical records at RSUD AlIhsan Bandung using total sampling from July to December 2023. The study results showed 6 cases (9.10%) of indications without therapy, 60 potential cases (90.90%) of adverse drug reactions, and no cases in the categories of therapy without indication, incorrect drug, or incorrect dosage. Based on laboratory tests of random blood glucose levels, 77.78% of patients achieved treatment targets while 22.22% did not. In terms of urea and creatinine levels, 55.55% showed effectiveness, while 44.45% did not show treatment effectiveness. The conclusion of this study is that there is a potential incidence of DRPs, with 6 cases (9.10%) of indications without therapy and 60 potential cases (90.90%) of adverse drug reactions. The effectiveness of therapy based on blood glucose levels showed 77.78% success, while urea and creatinine levels showed 55.55% effectiveness.
