Abstract

Hipertensi merupakan salah satu penyakit tidak menular dengan prevalensi tinggi dan menjadi faktor risiko utama penyakit kardiovaskular yang berdampak pada kualitas hidup pasien. Berdasarkan Riskesdas 2018 prevalensi hipertensi di Indonesia mencapai 34,1%, namun sebagian besar pasien tidak rutin menjalani terapi sehingga berpotensi menimbulkan Drug Related Problems (DRPs). DRPs adalah kejadian yang berkaitan dengan penggunaan obat dan dapat memengaruhi efektivitas maupun keamanan terapi. Penelitian ini bertujuan untuk mengetahui kejadian serta jenis DRPs pada pasien hipertensi di RSUD Welas Asih Provinsi Jawa Barat. Penelitian bersifat deskriptif noneksperimental dengan metode retrospektif menggunakan 95 rekam medis pasien hipertensi tahun 2024, dan identifikasi DRPs dilakukan menggunakan Pharmaceutical Care Network Europe (PCNE) V9.00. Hasil penelitian menunjukkan mayoritas pasien berjenis kelamin perempuan (74,7%) dengan rentang usia terbanyak 51–60 tahun (35,8%), sedangkan hipertensi tahap 2 menjadi diagnosis yang paling banyak ditemukan (61,1%). Terapi antihipertensi terbanyak berupa monoterapi (49,5%) dengan obat dominan amlodipine (38,9%). Jenis DRPs yang paling sering ditemukan adalah P2.1 kejadian obat merugikan (48 kasus), C3.1 dosis obat terlalu rendah (35 kasus), serta C1.4 kombinasi obat tidak tepat (17 kasus). Kesimpulan dari penelitian ini adalah masih tingginya kejadian DRPs pada pasien hipertensi, sehingga diperlukan evaluasi terapi secara berkesinambungan, menyeluruh, dan optimal guna meningkatkan efektivitas serta keamanan pengobatan. ------ Hypertension is a noncommunicable disease with a high prevalence and is a major risk factor for cardiovascular disease, which impacts patients' quality of life. Based on the 2018 Riskesdas, the prevalence of hypertension in Indonesia reached 34.1%, but most patients do not undergo regular therapy, which has the potential to cause Drug Related Problems (DRPs). DRPs are events related to the use of drugs and can affect the effectiveness and safety of therapy. This study aims to determine the incidence and types of DRPs in hypertensive patients at Welas Asih Regional General Hospital in West Java Province. This is a descriptive, non-experimental study using a retrospective method with 95 medical records of hypertensive patients in 2024, and DRPs were identified using the Pharmaceutical Care Network Europe (PCNE) V9.00. The results showed that the majority of patients were female (74.7%) with the most common age range being 51–60 years (35.8%), while stage 2 hypertension was the most common diagnosis (61.1%). The most common antihypertensive therapy was monotherapy (49.5%) with amlodipine as the dominant drug (38.9%). The most common types of DRPs were P2.1 adverse drug events (48 cases), C3.1 inadequate drug dosage (35 cases), and C1.4 inappropriate drug combinations (17 cases). The conclusion of this study is that the incidence of DRPs in hypertensive patients remains high, thus requiring continuous, comprehensive, and optimal evaluation of therapy to improve the effectiveness and safety of treatment.
