Abstract

Tuberkulosis (TB) merupakan penyakit menular yang masih menjadi masalah kesehatan masyarakat di Indonesia. Meskipun pengobatannya ditanggung pemerintah, perbedaan jenis regimen seperti Kombipak dan KDT (kombinasi dosis tetap) dapat mempengaruhi efektivitas terapi dan biaya yang dikeluarkan oleh fasilitas pelayanan kesehatan. Penelitian ini bertujuan untuk menganalisis efektivitas biaya antara regimen OAT Kombipak dan KDT pada pasien rawat jalan di salah satu RSUD Kota Bandung tahun 2024. Penelitian ini menggunakan metode kuantitatif deskriptif secara retrospektif dengan pendekatan farmakoekonomi berupa analisis cost-effectiveness. Sampel berjumlah 38 pasien dipilih menggunakan teknik total sampling berdasarkan kriteria inklusi. Data yang dikumpulkan meliputi biaya pengobatan langsung dan hasil efektivitas berdasarkan hasil BTA akhir. Hasil penelitian menunjukkan bahwa regimen KDT memiliki biaya rata-rata yang lebih rendah dan efektivitas lebih tinggi dibanding Kombipak. Hasil menunjukkan nilai Average Cost Effectiveness Ratio (ACER) pada terapi OAT KDT yaitu sebesar Rp 19.967 nilai tersebut lebih rendah dibandingkan dengan OAT Kombipak yaitu sebesar Rp 24.379, sehingga terapi OAT KDT lebih costeffective dibandingkan terapi Kombipak pada pasien TB kategori 1 rawat jalan. Hasil analisis sensitivitas dari nilai ACER menunjukkan terapi OAT Kombipak memiliki rentang yang paling panjang sehingga terapi OAT Kombipak lebih mempengaruhi nilai cost-effectiveness. ------ Tuberculosis (TB) is an infectious disease that remains a major public health concern in Indonesia. Although the treatment is funded by the government, differences in drug regimens such as Kombipak and KDT (fixed-dose combination) may affect therapeutic effectiveness and the treatment costs incurred by healthcare providers. This study aimed to analyze the cost-effectiveness of Kombipak and KDT regimens in category 1 outpatient TB patients at a public hospital in Bandung City in 2024. This study used a retrospective quantitative descriptive method with a pharmacoeconomic approach in the form of cost-effectiveness analysis. A total of 38 patients were selected using a total sampling technique based on inclusion criteria. The collected data included direct medical costs and treatment effectiveness, based on final AFB (Acid-Fast Bacilli) smear results. The findings showed that the KDT regimen had a lower average cost and higher effectiveness compared to the Kombipak regimen. The Average Cost-Effectiveness Ratio (ACER) for KDT was IDR 19,967, which was lower than Kombipak’s ACER of IDR 24,379, indicating that KDT was more cost-effective. Sensitivity analysis revealed that Kombipak had the widest cost range, suggesting greater variability in its costeffectiveness outcome.
